The Cardiovascular Connection site on the Healthier Washington Collaboration Portal was created to reach clinical and community partners with resources and information about improving cardiovascular health outcomes in our state, especially among populations experiencing health disparities.

This site is connected with the Washington State Cardiovascular Disease and Diabetes Network Leadership Team (CDNLT) and the Diabetes Connection, which provides resources to organizations and individuals working in Washington to control and prevent diabetes and cardiovascular disease.

The Washington State Department of Health provides support and staffing to this work through grants from the Centers for Disease Control and Prevention, which supports efforts to improve cardiovascular health in all states.
