Integrating Primary Care into Behavioral Health Settings

In Spring 2017, the Healthier Washington Practice Transformation Support Hub partnered with the University of Washington AIMS Center to provide a free learning series on Bi-Directional Integration for Behavioral Health Agencies and Primary Care Practices. The series was led by faculty and staff at the AIMS Center and guided practice teams through the core principles, the roles and responsibilities, the development of integrated workflows and identifying population tracking tools, and the process of implementing rapid cycle PDSAs for integrated care. 

The learning series was uniquely tailored to address integration in behavioral health or primary care settings, as such, it was divided into two tracks. Each PDF includes presentation slides and learning activities provided to participants of either the behavioral health track or primary care track.
